Medicare is not automatic for everyone. Whether you need to actively sign up β and when β depends on your specific situation. The most common mistake we see Midlands seniors make is assuming they do not need to do anything because they are "not retired yet" or because "someone will notify them." Neither assumption is reliable.
Here is the single most important rule: your Initial Enrollment Period (IEP) is a 7-month window centered on the month of your 65th birthday. It begins 3 months before your birthday month and ends 3 months after. If you miss this window without a qualifying reason, you may face a permanent late enrollment penalty and a gap in coverage that cannot be reversed.
We recommend calling us at least 4 months before your 65th birthday β that gives us time to walk through your situation, explain your options, and get everything set up before your coverage needs to begin.

Enrolling early (MarchβMay) means coverage starts June 1. Enrolling in June starts July 1. Enrolling after June adds delay and may cause a gap.
Our recommendation: Enroll in Parts A and B during the 3 months before your birthday month. This ensures your coverage starts on time with no gap. Then contact us to compare supplemental plans β Medicare Advantage or Medicare Supplement β before your coverage begins.
Many Midlands residents are still working at 65 and covered by an employer health plan β through their own employer or a spouse's. Your situation requires careful handling to avoid both overpaying and accidentally triggering late enrollment penalties.
If your employer has 20 or more employees, your group plan is primary and Medicare is secondary. You can delay Medicare Part B enrollment without penalty. However, you should still enroll in Part A if you qualify for it premium-free β it costs nothing and provides a safety net.
If your employer has fewer than 20 employees, Medicare becomes primary at 65. You need to enroll in Parts A and B on time or face potential coverage gaps and higher costs. Call us before your birthday to plan this carefully.
If your spouse is still working and you are covered under their employer plan, the same rules apply based on their employer's size. When that coverage ends, you typically have an 8-month Special Enrollment Period to enroll in Medicare with no penalty.
Hospital insurance. Covers inpatient hospital stays, skilled nursing facility care, hospice, and some home health care. Most people get Part A premium-free if they worked 40+ quarters.
Medical insurance. Covers outpatient care, doctor visits, preventive services, lab work, and durable medical equipment. The standard 2026 premium is $185/month, deducted from Social Security if you receive benefits.
Medicare Advantage. An all-in-one alternative offered by private carriers that replaces Parts A and B β often with drug, dental, and vision coverage bundled in. Subject to carrier networks.
Prescription drug coverage. Standalone plans that pair with Original Medicare, or bundled into Medicare Advantage plans. Enroll when first eligible to avoid the late enrollment penalty.
